A phased return to work is a way of enabling employees to return to their duties in a gradual way following illness or injury.  This pro-forma will hold both employer and employee accountable to an agreed temporary work pattern and/or an adjustment in duties, ensuring the transition back into role is correctly documented and within the agreed timeframe.
